TRIAL LESSON SIGN-UP

WAIVER & RELEASE

1. Assumption of Risk & Release of Liability

I understand that Taekwondo involves physical activity and risk of injury. I voluntarily allow my child to participate in a Trial Lesson at Tiger Cheon’s World Taekwondo and assume all risks. I release Tiger Cheon’s World Taekwondo, its instructors, staff, and volunteers from any liability for injuries or damages arising from participation.

2. Photo & Media Consent

I give permission for Tiger Cheon’s World Taekwondo to use photos or videos of my child from the Trial Lesson for promotional purposes, including social media and website use.

3. Parent/Guardian Acknowledgment

I understand that a parent or guardian must remain on-site during the Trial Lesson and that participation is voluntary.
